Community OB/GYNs and primary care: Many OB/GYN practices and family medicine doctors provide menopause management and will bill insurance if they participate with your plan. Call the clinic’s billing office and ask: Do you accept [your insurer]? Is Dr. X in-network for [plan name]? Do you bill for telehealth visits and labs? Will you submit claims for services and prescriptions? Ask your insurer: “Is [clinic name or NPI/doctor name] in-network for my plan? Will they cover visits and hormone therapy prescriptions?” Get CPT/diagnosis codes if you want pre-authorization or benefits verification (the clinic’s biller can provide these). Sample script to call a clinic or insurer To clinic: “Hi, I have [insurance name, plan type]. Does your clinic accept this insurance and is Dr. [name] in-network? If not, do you bill out-of-network or provide superbills for reimbursement?” To insurer: “Is [clinic or doctor name] in-network for my plan?
